The Québec Ombudsman pledges to protect the confidentiality of the information transmitted. Under the Act respecting access to documents held by public bodies and the Protection of personal information, the information obtained or created by the Québec Ombudsman in the context of verification or investigation as the result of disclosure of wrongdoing cannot be communicated.

Describe the act committed, by indicating, if possible:

Who

  • The name and address of the public body involved;
  • The name of the unit involved;
  • The name, function and contact information of the person who committed the act;
  • If the act consists of something you heard, the identity of the speaker;
  • If other people are involved or are aware of the act in question, their name, function and contact information.

What

  • A description of the act;
  • A description of the events.
  • If you would like to make an anonymous disclosure, please provide as much information as possible so that the verification or investigation can be completed.

When

  • If the act was already committed, specify when. If it is about to occur, please indicate it.

Where

  • Place where the act was committed.

Why

  • Why you consider that the act qualifies as wrongdoing.
